I live here in Mn and watch the twins constantly, that nickname was not widely used up here, as a matter of fact I'd never heard of it until recently. They certainly didn't mention it during broadcasts, either TV or radio.
Yep. Most MLB broadcasts go to commercial breaks when a reliever comes in, so the TV audience doesn't get to see these intros. The Phillies broadcast intentionally didn't go to a break, just like the Mets used to hold off on commercials for the Edwin Diaz entrances.
Not sure when it started, but I do remember the Padres always playing Hells Bells by AC/DC whenever Trevor Hoffman came in. That must've been 30 years ago.

The other week he said he injured his hand last season against the Royals and hasn't been the same since. You would think whatever injury he sustained last year would have been healed by now, but that's what he said, I'll try to find the article.

Betts recently spoke on what the cause of his career-worst slump might have been, and what he is doing to try and get back to form like the glimpse Dodgers fans saw on Friday.
“I really haven’t been right since I came back from my hand last year,” Betts said.
Betts broke his hand in the middle of last June and spent close to two months on the injured list out of commission.
“Think about it,” Betts said. “Go and look at it. I haven’t been right since.”
Betts played 44 regular season games after returning from the IL. He was batting .263 with an .811 OPS, compared to a .304 batting average before the injury with an OPS of .892.
This season, Betts has a .239 batting average and a hard-hit rate (batted balls with an exit velocity of 95mph or faster) of just 33.2 percent, putting him in the 16th percentile in MLB.
“I wasn’t blaming it on my hand or anything,” he said. “I was just saying since coming back, I haven’t done anything. It’s not just this season.

The gambling scandal he was completely exonerated, all the facts pointed to his interpreter and there was no evidence implicating him at all. I don't know what this Hawaii thing is, but just because someone is accused of wrongdoing doesn't make it so, let's see how the story plays out and what the actual facts are. Whenever Trump is accused of wrongdoing, something that has happened hundreds of times, the alt right always claim it's fake news and a hoax, so before jumping to any conclusions, let's see the facts laid out and what the investigation turns up, I'm willing to give him the benefit of the doubt unless the facts prove otherwise
